Anvian Health provides nurse-led, neuroaffirming support that complements primary care and allied health services.
We work alongside GPs, specialists, clinics, support coordinators and community providers to improve patient understanding, follow-through and system navigation.
Our role is to complement the care you are providing.
We welcome referrals from:
• General Practitioners
• Mental health care providers
• Allied health professionals
• Specialist physicians
• Community health teams
• NDIS support coordinators
• Rural and regional clinics
• Non Government Organisations
We support patients who require structured, ongoing guidance outside of brief appointment windows.
Mental Health Nursing Support
We provide structured, nurse-led support to assist clients in managing their mental health and overall wellbeing, in collaboration with their broader care team.
Our role may include:
• Monitoring changes in wellbeing, symptoms, and daily functioning
• Supporting the development of personalised care plans and wellbeing strategies
• Providing evidence-informed psychoeducation in plain language
• Assisting clients to identify practical strategies to support daily functioning and emotional regulation
• Supporting preparation for appointments and communication with GPs, psychologists, and other providers
• Facilitating coordination and continuity of care across services
We support clients to build stability and confidence between appointments.
We do not provide psychological therapy or replace specialist mental health care.
Our role is to provide nursing support, education, monitoring, and care coordination within scope, and to work collaboratively with the client’s treating team.
Medication Literacy Support
We provide education and support to help clients better understand their prescribed medications and participate confidently in conversations with their treating clinician.
Our role may include supporting clients to:
• Understand the purpose of their prescribed medications using consumer medicine information and plain-language explanations
• Recognise and document potential side effects or changes to discuss with their prescribing clinician
• Prepare questions for medication reviews with their GP, pharmacist, or specialist
• Build confidence to participate in informed discussions about their treatment plan
We do not prescribe, alter, or recommend medication changes.
Our role is to support understanding, safe use, and appropriate follow-up with the prescribing healthcare professional.
Chronic Condition Support
Many patients with chronic illness require coordination, organisation and clarification that exceeds standard consult time.
We support:
• Care planning and system navigation
• Appointment preparation
• Executive functioning strategies
• Health history mapping
• Communication support
This improves clarity, adherence and patient confidence.
Neurodivergent and NDIS Support
We provide neuroaffirming support for individuals that have a formal diagnosis or identify as meeting the medical model criteria for Autism, ADHD or other categories of neuruodivergence
Supports include:
• Pre- mid and post-diagnosis guidance
• NDIS access n support
• Preparation for assessments and reviews
• Executive functionin education
• healthy literacy support
We help clients make sense of their healthcare advice and turn it into practical next steps.
Patients who may benefit from referral include:
• individuals or families overwhelmed by the process within our healthcare systems
• Have complex mental health or chronic condition presentations
• Require structured support between medical appointments
• Experience challenges with executive functioning
• Need assistance preparing for documentation for access to NDIS, My Aged Care etc
We focus on capacity-building and clarity.
This is not a suitable service if your patient requires:
Acute crisis response
• Emergency care
• Inpatient services
• Procedural Nursing
• Medication Prescrioptions
We operate within a collaborative, community-based framework in line with AHPRA and NMBA standards - insert link please
We prioritise structured communication with referring providers.
We will collaborate with referrers to ensure tha tht epatient receives the appropriate cl.inical care, colloabratiiive communication may include:
• Progress reports where clinically appropriate
Support framework summaries and reccomendations
Ongoing management plans
